UnityPoint Health PACE Senior Care in Hiawatha opening June 1, now enrolling participants

UnityPoint Health has announced that its new Program of All-Inclusive Care for the Elderly (PACE) Senior Care Center in Hiawatha is now accepting enrollment applications ahead of its official opening June 1.

Located at 1450 N. Center Point Road in Hiawatha, the PACE Senior Care Center center aims to help eligible older adults remain in their homes by offering coordinated medical and support services. The program provides a comprehensive range of care, including primary and specialty medical services, therapy, prescription medications, transportation, and in-home support.

PACE services include primary care through an on-site clinic, rehabilitation therapies, durable medical equipment, respite care, emergency services, hospitalization, and dietary support.

Additional offerings include social work services, therapeutic recreation, and free interpretation services. Transportation to and from the PACE center or other approved medical appointments in the community is also included.

To qualify for the PACE program, participants must be 55 or older, require a nursing facility level of care, live in one of the eight eligible counties – Benton, Buchanan, Cedar, Delaware, Iowa, Johnson, Jones or Linn – and be able to live safely in the community at the time of enrollment.

UnityPoint Health said eligibility and enrollment are based on assessments conducted in coordination with the Iowa Department of Health and Human Services. There are no deductibles or copays for services approved by the program’s interdisciplinary team. Premiums vary depending on participants’ Medicare and Medicaid status.

The program is designed to improve quality of life and access to care for older adults. According to UnityPoint Health, the goal is to help seniors live independently for as long as possible through personalized and comprehensive care planning.
